Patient Responsibilities

PATIENTS ARE RESPONSIBLE FOR:

ACCURATE INFORMATION
Report changes in health status. Give accurate and complete dental and medical information. For example, it is important to inform your provider PRIOR to x-rays or treatment, if you’re pregnant or think you may be pregnant, or if you had ANY significant changes in your medical status since your last visit.

RESPECT
Be respectful of EmblemHealth Family Dental Practice office personnel and other patients as well as office property.

KEEPING APPOINTMENTS
Attend scheduled appointments promptly or call to make necessary changes in appointments as early as possible.

PAYING BILLS
Promptly pay all charges for treatment received. Accept personal responsibility for all charges not covered by insurance.

FOLLOWING RECOMMENDATIONS
Help maintain personal dental health. Participate in your care, follow the practitioner’s oral hygiene recommendations and follow the treatment plan prescribed by your provider.

CHILD and DEPENDENT PATIENT CARE
Make arrangements for the care of small children or dependent adults during dental appointments and, when necessary, provide a responsible adult to transport the patient home from the facility and be available to him/her for post-operative care.
