The Information We Collect

Use of the Information this Site Gathers and Tracks

The Entity That Maintains Personally-Identifiable Information

Unless specifically noted, all personally identifiable information presented on this site is retained by EmblemHealth. The information that is collected via EmblemHealth's Health Risk Assessment (HRA) is maintained by our HRA partner, Protocol Driven Health Care, Inc. (PDHI).

Personal Identifiable Claims Information Retention

Member claims information is retained on the secure section of our web site for 18 months after a member has terminated their coverage with EmblemHealth. After 18 months, that member's claim information will no longer be accessible through EmblemHealth's web site. A record of the member's termination date and eligibility status will be permanently available online for reference.

Making Changes to the Information You Provide Us

At any time, you can change, update or remove from our system any information you have voluntarily provided.

Sites To Which We Link

If you choose to use any of the links we provide to other organizations' sites you will be leaving our web site and going to a new one. We will inform you when you are leaving our site. Protection of privacy at those other sites is governed by the privacy policy at that site.
