Pre-admission and Post-discharge Call Program

Coming home from the hospital can be a stressful time. To help make that transition as smooth as possible, we offer a pre-admission and post-discharge call program. This program is offered free to members who get treatment at the following physician group practices:

A registered nurse will call you before you go into the hospital and again after you are sent home. The nurse will complete an assessment in order to help you get ready for your hospital stay and see what help you may need for when you leave the hospital.

Your assessment will also determine your need for extra services after you go home from the hospital such as:

  • Home care
  • Case management

Your nurse will work together in-person, by phone or by e-mail with your doctor(s). This helps make sure that your doctor(s) has all the necessary information to provide you with the highest quality of care possible. If you have multiple doctors treating you, your nurse helps all your doctors to communicate with each other to make sure your care is well coordinated.
