EmblemHealth’s Local Fruit & Veggies Program

EmblemHealth is offering the Local Fruit & Veggies Program to our 2016 Select Care and Essential Plan individual direct-pay members.

As a program member, you’ll be eligible to receive Health Bucks* worth up to $140 (for individuals) or $240 (for families)**. Health Bucks are coupons you can use toward fresh fruit and vegetables at New York City-based participating farmers markets.

Participation is limited, so be sure to sign up soon.

1. Join the Program
From July 15 to August 15, visit EmblemHealth Neighborhood Care to sign up for the program. A health care solutions specialist will help you register and give you your first set of Health Bucks. Note: Registration may end sooner than August 15 if program spaces fill up, so sign up as soon as you can.

2. Use Your Health Bucks
From July 15 to November 15, use all your Health Bucks to get fresh fruit and vegetables at any of the more than 140 participating farmers markets throughout New York City.

3. Pick Up Your Health Bucks Each Month
As long as you complete the program in full, you can pick up a total of 4 sets of Health Bucks at any EmblemHealth Neighborhood Care site, within the date ranges below. The dollar value increases each time:

4. Meet Program Requirements
You must take certain healthy steps to receive your Health Bucks and continue to participate:

A. Take Healthy Steps
To receive your Health Bucks each month by August 15, September 15 and October 15, the Select Care or Essential Plan subscriber (the main person on the health insurance policy) must complete at least one healthy activity*** at EmblemHealth Neighborhood Care either before picking up your Health Bucks or the day you pick them up.

B. See Your Doctor
To receive your last set of Health Bucks by October 15, the Select Care or Essential Plan subscriber must have had your no-extra-cost annual preventive care checkup with your primary care physician (PCP) if you haven’t had this appointment since January 1, 2016****. Complete the Confirmation of Checkup form when seeing your doctor; then bring it with you the next time you pick up your Health Bucks.

About Farmers Markets
Farmers markets are community sites where farmers, fishers and bakers sell their locally produced goods. They are also spaces for neighbors to share cooking tips, take market tours, and learn about farming and gardening.

* Health Bucks are developed by the NYC Department of Health and Mental Hygiene. EmblemHealth offers Health Bucks as a community organization. Health Bucks cannot be used as or exchanged for any US or international currency (money).

** For the purposes of this program, an “individual” is the plan subscriber — the main person on the health insurance policy — and a “family” is the plan subscriber plus at least one other family member on the plan.

*** If you think you might be unable to perform a healthy activity due to your medical condition, you might qualify for an opportunity to participate in this program through different means. Contact us at any Neighborhood Care location and we will work with you (and if you wish, your doctor) to find a reasonable alternative healthy activity that is right for you in light of your health status.

**** Non-preventive care services received from an in-network health care provider may be subject to any applicable copay, coinsurance or deductible.
