Healthy Living PATH Program 24 Hour Health Information Line

We offer our members the 24 Hour Health information Line in partnership with Alere. The 24 Hour Health information line is staffed by highly trained registered nurses. They will guide you in making informed decisions about everyday health issues and how best to handle a medical concern.

The program also helps as an "early warning system" for health conditions before they may need a doctor visit, emergency room visit or hospitalization.

Members can call the 24 Hour Nurse Line at 1-877-444-7988, 24 hours/seven days a week. TTY/TDD users please call 711.

Vital Decisions – Living Well Program

At EmblemHealth, we understand the importance of maintaining personal dignity and a sense of control and well-being, even in times of crisis. We offer our Medicare members the Vital Decisions–Living Well Program in partnership with Dignified Decisions. This program supports and guides members, their families, caregivers, as well as, providers through the difficult choices at a time of advanced illness. A program specialist will work with members and their families to prepare a smooth transition to hospice care.

EmblemHealth PATH programs

Managing a condition can be overwhelming. We are here to assist by offering eligible members access to education and support through our PATH health management programs. These programs complement the care provided by your doctor by helping you better understand and manage your condition and meet your health goals. They help members manage their chronic health conditions including diabetes, Chronic Obstructive Pulmonary Disease, asthma, heart failure, Coronary Artery Disease, hypertension and depression. These programs work to help members take control of their health.

Our PATH Programs are designed to:

  • Help you understand your condition.
  • Improve your discussions about your health and safety with your doctors.
  • Show you how to make lifestyle changes that will keep your condition under good control.
  • Identify ways for you to stay healthy and safe.
  • Coordinate your treatment plan with your doctor.
  • Give you a chance to tell us about your experience with the program.

When you enroll in one of our programs you may:

  • Work one-on-one with a professional health coach or nurse by telephone.
  • Get free educational materials about symptom management, health and safety risks, treatments, diet and nutrition and more.
  • Find support services provided in your community.
