There is Help in Controlling Diabetes

Controlling DiabetesIn March, 2015, Harlem and Cambria Heights EmblemHealth Neighborhood Care launched a potentially life-changing program — the “Six-Week Diabetes Self-Management Program.” The class is offered to people with diabetes and their caregivers. For six weeks, 12-15 participants spend approximately 2.5 hours learning about many of the issues they confront living with diabetes. The class is facilitated by two trained Neighborhood Care staff members.

Addressing the Real-Life Issues Faced by People with Diabetes

Topics offer techniques for dealing with the symptoms of diabetes and include:

  1. Fatigue
  2. Pain
  3. Hyperglycemia/Hypoglycemia
  4. Stress and emotional problems such as:
    • Depression
    • Anger
    • Fear
    • Frustration

This new program joins a suite of health programs offered by Neighborhood Care including the National Diabetes Prevention Program — that targets those who are at risk for developing the disease — before it strikes to keep our members and communities healthy. These are some of the topics discussed in this program:

  • Exercise
  • Healthy eating
  • Appropriate use of medications
  • Effectively working with health care providers
