Primary Care for Patients with HIV/AIDS

Primary Care HIV/AIDSWhen your patients living with HIV/AIDS seek routine primary care services, provide clear instructions about the care they should get each year, including:

  • One outpatient visit every six months (one in the first half of the year and another in the second half of the year) for ongoing primary care for their HIV and preventive health care needs.
  • One viral load test every six months (one in the first half of the year and another in the second half of the year).
  • One syphilis test annually.

Patients with HIV typically require assistance with coordination of services. Our Complex Case Management department offers an HIV Case Management program to help patients with HIV identify and obtain care, learn about disease process and management, adhere to complex medication schedules and find community resources.

The EmblemHealth HIV Case Management program is staffed by specially trained HIV case managers. To refer a patient to this program, please call or have the patient call 1-800-447-0768.
