Introducing: HIP VIP Dual Eligible (HMO) Medicaid Advantage Plus-Managed Long Term Care Program

We are pleased to announce the launch of the HIP VIP Dual Eligible (HMO) Medicaid Advantage Plus-Managed Long Term Care (MAP-MLTC) plan. The HIP MAP-MLTC plan is designed for individuals who are eligible for both Medicare and Medicaid and who need care services that would otherwise be provided in a nursing home.

As of September 1, 2010, the HIP MAP-MLTC plan is being offered in the five New York City boroughs, and Westchester, Nassau and Suffolk counties.

What is HIP MAP-MLTC?
The goal of the MAP-MLTC program is to assist frail, elderly and/or disabled individuals so that they can stay in their homes for as long as possible. In addition to their existing Medicare benefits, members of the MAP-MLTC program can receive additional services such as:

  • Adult day care and social day care programs
  • Personal emergency response systems
  • Home-delivered and congregate meals
  • Personal care services to assist with bathing, eating and dressing

Every HIP MAP-MLTC member will be assigned a nurse care manager who will work with our network providers to develop a personalized plan of care. Our nurse care managers will be contacting health care practitioners who we have identified as having treated members who are eligible for MLTC participation. If you are one of these practitioners, we ask that you work with our care managers to develop a care plan that offers your patients the most robust course of care.
