Managing a condition can be overwhelming. We are here to assist by offering eligible members access to education and support through our PATH health management programs. These programs complement the care provided by your doctor by helping you better understand and manage your condition and meet your health goals.

Mental Health and Substance Abuse
EmblemHealth offers a range of mental health and substance abuse resources and information to help you stay healthy, both physically and mentally. Doctors can help detect depression and other mental disorders in their teen patients ages 11 to 18 by participating in Columbia University’s Teen Screen Program, a recognized national model for teen mental health screening.
Integrative Wellness Programs

Provides information, resources and tools to help our members and others manage the challenges of caregiving and encourage self-care.
Hospital Visitation Program
Recruits and trains volunteers who offer comfort and support to hospitalized members to nurture the spirit and foster healing.
Dignified Decisions — End of Life Care Program
Facilitates informed decisions by members, their families and physicians at a time of advanced illness while assuring personal dignity and a good quality of life.
Personal Health Improvement Program (PHIP)
Provides the tools — through lectures, group exercises, and home learning — to combat stress, pain or discomfort and embrace a healthier lifestyle.
