9/11 Health Insurance Group Benefits Program

The 9/11 Health Insurance Group and EmblemHealth are working together to offer a health benefits program to widows, widowers and their dependent children who lost health insurance coverage as a direct result of the events of
September 11, 2001. The 9/11 Health Insurance Group Benefits Program, underwritten by GHI, an EmblemHealth company, provides a broad range of medical, hospital and prescription drug benefits.

The plan offers:

• A Network of Participating Doctors. Our network includes access to leading doctors and other practitioners throughout the tristate area and upstate New York, as well as most acute care hospitals in the region.
• Specialist Visits Without Referrals. Because the 9/11 Health Insurance Group Benefits Program is a PPO, you can see any of our network doctors – including specialists – anytime you need care, for just a small copay. You don’t need another doctor to refer you.
• Out-of-Network Coverage. You can also receive most covered services from out-of-network doctors and other practitioners, subject to the plan’s deductible and coinsurance, although this will increase your out-of-pocket expenses. You are also covered for emergency services in any hospital nationwide.
