Healthy NY Policy Changes
We want to make your administrative experience with us as easy and efficient as possible. That is why we have changed the process for submitting Healthy NY applications. This process is now consistent with the application process that applies to other EmblemHealth community-rated individual and small group plans.
Please note EmblemHealth's modified process for receiving and reviewing Healthy NY applications, which is effective immediately:
- Legible and complete applications must be received by the 20th of the month prior to the desired effective date. Please mail completed applications to the address below:

- We will no longer accept e-mailed applications. We require original signatures for each application. Please mail your applications to the above address.
- Representatives processing Healthy NY cases will no longer call or e-mail brokers or prospective members to request required documentation that is missing from the application.
- Applications that come in illegible or are missing necessary documentation will be mailed back to the applicant with a letter explaining what is missing and the steps the applicant must take for the application to be processed.
We thank you in advance for adhering to these policy changes. Together we can help to ensure that Healthy NY applications are processed timely and accurately.
Note: These policy changes are in affect for all Healthy NY plans, including those that will be discontinued on December 31, 2013 and the small employer Healthy NY plan that will take effect January 1, 2014.
