Invite your patients to take a tour of one of our locations and then discuss their health care issues with our nurses, pharmacists and care navigators. They can also discuss referrals, learn more about preventive care and find out about treatment options for existing conditions. We can also help with ways to lower out-of-pocket costs, resolve billing issues, assist with joining health and wellness programs, and connect you to community support.

The CDC’s Injury Center created the Stopping Elderly Accidents, Death and Injury (STEADI) Toolkit, a free resource guide to assist health care providers with the tools they need to assess/address their patients’ fall risk. This guide also helps with fall prevention conversations with your patients. The STEADI Toolkit can be downloaded or ordered online.

Keeping Seniors on Their Feet

EmblemHealth is actively trying to help older members stay “on their feet.” Our Steps-4-Safety PATH program is available to Medicare and Dual Eligible Special Needs Plan (SNP) members who are considered to be at risk for a fall, or may have already suffered a fall or fracture.
