EmblemHealth Special Needs Plans (SNPs)
If you are in a Medicare savings plan, or you are receiving assistance from the state and living in our service area, we have Special Needs Plans (SNPs) for you. We understand you need health care coverage that does everything possible to help you stay healthy at a price you can afford. Our plans provide you with all the benefits of Original Medicare, with a lot of additional benefits, often with little or no out-of-pocket costs.
Dual Eligible (PPO SNP)
A plan like PPO II, designed for people who are Medicare eligible and are receiving additional State assistance, or who are in a Medicare savings program. This plan offers in and out of network coverage.
Dual Eligible (HMO SNP)
A plan like VIP (HMO), designed for people who are Medicare eligible and are receiving additional State assistance, or who are in a Medicare savings program. This plan offers in network coverage.

- $0 annual out-of-pocket limit (PPO plan only)
- $0 copay for inpatient hospital care
- $0 copay for primary care physician office visits
- $0 copay for podiatry office visits

Low-Income Subsidy Information
