It pays to be active — really!
If you’re a member of an EmblemHealth Qualified Health Plan, Essential Plan, small group plan, or certain large group/employer* plan, you can take advantage of our ExerciseRewards™ program.
When you participate in the ExerciseRewards program, you can earn a $200 reward each six months in a calendar year. Your eligible spouse and/or dependent(s) can earn a $100 reward.
Here’s how the program works:
- Work out at a qualifying fitness center or exercise center** at least 50 days during each six-month period in a calendar year (January 1 through June 30; July 1 through December 31).
- Choose one of the three ways below to keep track of and submit your progress.
- When you complete your required visits each six months, based on a calendar year, you’ll receive a check in the mail for $200. When your eligible spouse and/or dependent(s) completes their required visits, they’ll receive a check in the mail for $100.

This app uses global positioning system (GPS) data from your smartphone to track your visits in real time at over 41,000 fitness centers and select YMCAs nationwide.
Your visits will be tracked through your ExerciseRewards account, as long as each workout session is at least 30 minutes long. Note: The app will count only one check-in per calendar day toward your reward.
You can download the ASHConnect app in your app store. Purchase of a device is not included with your participation in the ExerciseRewards program.
You can manually track your fitness center visits on the Visit Submission Form.
Your qualifying fitness center must approve and sign your Visit Submission Form or give you a printout of your visit. It is your responsibility to submit your Visit Submission Form to the ExerciseRewards program.
- Your Visit Submission Form must be received no later than 90 days after the end of each benefit period.
- You must redeem your reward on the ExerciseRewards program website.
- All rewards are processed within 14 days of redeeming your reward.
The Active&Fit Direct™ program
The Active&Fit Direct program is an optional program that provides access to participating fitness centers and select YMCAs. It is not covered under your insurance plan.
The Active&Fit Direct program lets you choose from over 10,000 participating fitness centers and select YMCAs nationwide. It costs $25 a month, plus a $25 enrollment fee and applicable taxes.
Once you’re enrolled, the Active&Fit Direct participating fitness centers will automatically submit your visits to the ExerciseRewards program.

*Gym exercise rewards program benefit is included in all EmblemHealth HMO Plus and EmblemHealth EPO Value plans. For other EmblemHealth large group/employer plans, check your Certificate of Insurance or Wellness Program provisions to see if your plan has added the gym exercise rewards program benefit. The ExerciseRewards and Active&Fit Direct programs are provided by American Specialty Health Fitness, Inc. (ASH Fitness), a subsidiary of American Specialty Health Incorporated (ASH). ExerciseRewards, Active&Fit Direct, and ASHConnect are trademarks of ASH and used with permission herein. Members are not required to participate at an ASH-contracted fitness center to be eligible for the reward; however, exclusions apply. ExerciseRewards and Active&Fit Direct are health improvement and education programs and are not insurance. ASH Fitness is a separate company that administers the ExerciseRewards and Active&Fit Direct programs on behalf of EmblemHealth.
**For a fitness center to qualify, it must be located in the 50 U.S. states or District of Columbia, and offer regular cardiovascular, flexibility, and/or resistance training exercise programs. These exercise programs may include instructor-led classes (such as Zumba®, Pilates, “step” classes, yoga, aquatics, etc.). A qualifying fitness center must also have staff oversight, and offer a membership agreement. Only one exercise session may be logged per calendar day. There must be at least 8 hours between sessions.
