The dosage approved by the United States Food and Drug Administration for testosterone pellet replacement therapy in androgen-deficient males is 150 mg to 450 mg subcutaneously every three to six months. However, what is considered a normal testosterone level is highly variable and depends on hormone storage.

EmblemHealth will not allow payment for more than 450 mg (six 75 mg pellets) (the upper range of the standard dosage) of testosterone, unless:

  • There is information accompanying the claim that sufficiently documents that a higher than standard dose is needed
  • Symptoms of androgen deficiency accompanied by a subtherapeutic testosterone blood level are present
