EmblemHealth Presents Diabetes Program at HHS Webinar
The US Department of Health and Human Services (HHS) selected EmblemHealth to present our community-based National Diabetes Prevention Program (DPP) at a webinar entitled, "A Healthy People 2020 Progress Review," held on September 29, 2014.
Diabetes affects an estimated 29.1 million people in the United States and is the seventh leading cause of death. While effective therapies can prevent or delay diabetic complications, almost 25 percent of Americans with diabetes are undiagnosed*.

Neighborhood Care – Providing Member-Centric Support
The DPP, in partnership with the CDC and America’s Health Insurance Plans (AHIP), was launched in July 2013 to members at risk for diabetes. EmblemHealth is unique in that it is the only health plan in New York that directly provides this program to their members. EmblemHealth Neighborhood Care was the ideal place to implement the DPP since it’s located in communities with a high prevalence of obesity and diabetes. Additionally, as a result of our affiliation with AdvantageCare Physicians (ACP), we were able to obtain direct referrals from them, implement a multifaceted marketing approach and develop engaging collateral.
A CDC-trained EmblemHealth lifestyle coach led group sessions, which were held at EmblemHealth Neighborhood Care. Since member engagement is not one-size-fits-all, lifestyle coaches use a member-centric approach that provides an exceptional customer experience. The program demonstrates our commitment to finding innovative ways to care for our members. One participant said, “I’ve never had someone care about my personal health so much before.”
Robust Participation in EmblemHealth's DPP
To date, a total of 270 members enrolled in the year-long, evidence-based program and 203 members attended more than one class, reflecting a significant engagement rate of 75 percent. The positive results of the program showed 75 percent of the participants lowered their HbA1c, three out of four lost or maintained their weight and 40 percent improved their blood pressure.

Being selected by the HHS to present EmblemHealth’s DPP model is quite an honor. There are hundreds of DPPs across the United States, from which the HHS could choose.
