Kidney Care PATH Program
Our Kidney Care PATH program provides coordinated disease management to help supplement the care you give our members who have significant renal dysfunction. Kidney Care PATH was developed to help you manage your most complex cases and help your patients understand what they need to do to improve their condition. We encourage you to recommend one of these programs to your patients with kidney disease.
Program 1: Stage 4 or 5 Chronic Kidney Disease
Those with Stage 4 or 5 chronic kidney disease (CKD, filtration rates < 30cc/min not requiring maintenance dialysis) are offered a comprehensive service that provides guidance to improve compliance with your care plan. The focus of this program is to help your patients make lifestyle changes to improve their prognosis.
Interventions are derived from National Kidney Foundation's KDOQI, evidence-based guidelines and include:
- Timely referral to a nephrologist
- Dietary management
- Reversible cardiac risk factor mitigation
- Co-morbidity management
- Preemptive transplantation
- Dialysis modality education (where appropriate)
Program 2: End-Stage Renal Disease
Patients who have end-stage renal disease (ESRD) are offered an alternative, KDOQI-based program, focusing on the distinct issues that dialysis dependency. A partial list of the program services includes:
- Anemia management
- Dialysis adequacy
- Access selection and maintenance
- Bone mineral metabolism management
- Timely kidney transplantation
The clinical program is led by a full time, American Board of Internal Medicine (ABIM) certified nephrologist. Both programs are delivered telephonically by nephrology-trained nurses, with dialysis experience.
