New York State civil service employees can sign in to their EmblemHealth portal at my.emblemhealth.com to view their plan documents.

Select Benefits and Spending from the menu, and click on the Plan Documents link.

You can call New York State Dept. of Civil Service Program Services at 800-833-4344 for further information on your benefits or visit NYS Civil Service.

Select "I am a New York State Active Employee (NY)".
Select your group and plan.
You will be asked to sign in to view plan-specific benefits.
