We hope your new year is off to a great start! January is always a busy month as members take advantage of their new benefits. Scheduling timely appointments and helping our members get quality care is vital, and we would appreciate your extra efforts here. We invite you to take advantage of the following key resources to help you get a good start to 2023:
Finding a Member’s Copay Information
Our thanks to those who responded to our Fall 2022 Survey. Results showed some of you are having trouble finding a member’s copay information in the provider portal. Here’s where you can find it:
When you look up a member’s eligibility, use the Benefits Summary button on the Member Details page. Clicking this button opens a document that shows copayment information for the most commonly used services including, but not limited to:
- Hospital care – inpatient, outpatient, emergency room, urgent care, and rehabilitation.
- Medical care – primary care, specialty care, gynecology, chiropractic, acupuncture, and vaccinations.
- Diagnostic tests – radiology and laboratory.

Hello colleagues,
This month, I want to share our perspective on social determinants of health (SDOH). SDOH is a term that covers so many areas. SDOH are the non-medical factors that influence health outcomes. They are the conditions of the environments where people are born, live, learn, work, play, worship, and age. They include:
- Economic stability.
- Education access and quality.
- Neighborhood and environment.
- Health care access and quality.
- Community and social supports.*

Addressing SDOH at a population health level is complex and challenging. (Think of things like federal, state, and local politics; regulations; budgets; etc.) But we have tools to help you and your patients.
SDOH account for 30% to 55% of our health outcomes (World Health Organization [WHO], 2022). That means keeping patients healthy goes beyond identifying and treating illness and making recommendations for preventive care such as diet and exercise. As important as those things are, we also want to support you in understanding and addressing the bigger picture. For example, a recommendation to eat better cannot be followed by someone who cannot get healthy food.
SDOH is so important in the regulatory aspects of health plans, NCQA has added a new HEDIS measure for screening and providing interventions for transportation, food, and housing.
There is so much to say on this topic. Next month, we will begin to look at our three-pronged approach to SDOH: identify the barrier, listen to members and collect information, and identify reliable solutions.

Medicare Updates
New Requirement for Medicare Providers Caring for Members in Special Needs Plans
Beginning Jan. 1, 2023, EmblemHealth and ConnectiCare Medicare providers caring for Special Needs Plan members will be required to obtain the member’s consent for face-to-face virtual encounters. For full details, visit the Medicare provider resource hub.
Do Not Bill Members with Full Medicaid or QMB
If Medicare-Medicaid dual-eligible individuals have their Part A and Part B cost-share fully covered by their Medicaid plan, or are Qualified Medicare Beneficiaries (QMB), they are not responsible for their Medicare Advantage cost-share for covered services. Do not balance bill these members for any other costs. Any Medicare and Medicaid payments for services given to these members must be accepted as payment in full.
For EmblemHealth members, you can use ePaces to check whether the member has full or partial Medicaid benefits. For more detail, see EmblemHealth Medicare Advantage Plans.
To perform the same verification for ConnectiCare members, you can visit CT Department of Social Services or call 800-842-8440. For more detail, see ConnectiCare Medicare Advantage Plans.
NY Medicaid, HARP, and Child Health Plus Updates
Reminder: No Referrals for EmblemHealth’s NYS Sponsored Plans in 2023
Starting Jan. 1, 2023, EmblemHealth will no longer require primary care provider (PCP) referrals for specialty care for Medicaid (Enhanced Care), HARP (Enhanced Care Plus), and Child Health Plus (CHPlus) members. Additionally, EmblemHealth CHPlus member ID cards will be updated and reissued to reflect this change.
Gambling Disorder Treatment
Starting Jan. 1, 2023, EmblemHealth will cover gambling disorder treatment for Medicaid and HARP members.
eMedNY Provider Enrollment Portal Offers New Function
The eMedNY provider enrollment portal now allows practitioners to update expiring license data.
Change of Address (and Contact) Notification
Providers must notify Medicaid of any change of address, telephone number, or other pertinent information within 15 days of the change. For more information on this requirement and how to submit changes, see Reminder: Keep Your Directory Data Current.

Reimbursement Policy Updates
New Reimbursement Policy
We have formalized our Bundled Services Reimbursement Policy (Commercial and Medicare), which applies to both EmblemHealth and ConnectiCare. There are no changes to how claims are being paid. New codes are being added March 1, 2023. See the policy’s Revision History for the new codes.
Updated Reimbursement Policies
The following reimbursement policies are being updated. See their revision histories for effective dates and applicable changes:
- Home Health Care Services Reimbursement Policy
- EmblemHealth Laboratory/Venipuncture Reimbursement Policy
- Preventive Lists:
- Ambulatory Surgical Grouper Policy: ConnectiCare updated the ASC Grouper Policy to include new codes effective Jan. 1, 2023. Ambulatory surgical groupers will be paid according to surgical contracted rates when billed with revenue codes 360 or 490. If surgical services are billed with revenue codes other than 360 or 490 and the claims contain charges for anesthesia and/or recovery room, claims will be paid according to the surgical contracted rates unless otherwise negotiated.
Reminders: Reimbursement Policies and Programs Starting in January 2023
As a followup to our story in last month’s newsletter, starting Jan. 1, 2023, we introduced the following updates to our reimbursement policies:
- A new Foot Care Services and Nail Debridement Reimbursement Policy (commercial, Medicare, and Medicaid) for both EmblemHealth and ConnectiCare.
- The EmblemHealth and ConnectiCare Modifier Reference Policy will align with Centers for Medicare & Medicaid Services’ (CMS) 10% payment reduction for claims submitted with modifiers FX (use of traditional x-ray film rather than digital radiography) and FY (x-rays taken using computed radiography technology/cassette-based imaging).
- ConnectiCare’s Commercial Preventive Services List will be updated with State of Connecticut’s new breast/ovarian cancer mandate criteria.
- The Coding Edits reimbursement policies will include new edits applicable to modifier JW (drug amount discarded/not administered to any patient) (EmblemHealth | ConnectiCare).
- A new From/To Date Span Policy, Facility and Professional
(Commercial, Medicare, and Medicaid) Reimbursement Policy is being implemented to ensure we are only paying claims with properly documented dates of service. If the date of service submitted for a procedure is out of the “From” and “To” date calendar range or not in the correct format, claims will be denied.

February is National Children’s Dental Health Month
First Tooth. First Birthday. First Dental Visit.
February is National Children’s Dental Health Month and a good time to talk to your patients about the importance of taking care of their child’s dental health. The American Association of Pediatric Dentists recommends that parents establish a dental home for their child by their first tooth or first birthday.
The American Dental Association (ADA) offers resources such as the February 2023 National Children’s Dental Health Month (NCDHM) website and Program Planning Guide. Help promote the benefits of good oral health and prevention to children and adults, caregivers, teachers, and many others.
Here are some updates for providers about National Committee for Quality Assurance (NCQA) 2023 changes to the dental measures:
RETIRED - Annual Dental Visit (ADV). This measure focused on access to dental care, rather than quality of dental care.
NEW - Oral Evaluation, Dental Services (OED). Medicaid members under 21 years of age who received a comprehensive or periodic oral evaluation with a dental provider.
Intent: Good oral health is a vital component of a child’s overall health, and oral examinations are important to prevent disease, reverse disease processes, prevent progression of caries, and reduce incidence of future lesions. This measure will allow plans to understand if their pediatric members are receiving dental care and to work towards improving access and utilization of dental evaluations.
New-Topical Fluoride for Children (TFC). Medicaid members one to four years of age who received at least two fluoride varnish applications. Can be applied at the PCP office.
Intent: Dental caries is the most common chronic disease in children in the United States. Topical fluoride plays an important role in preventing tooth decay. This measure will allow plans to understand if their pediatric members are receiving fluoride varnish applications and to promote fluoride varnish treatments for their younger members.
Preauthorization List Updates
The EmblemHealth Preauthorization List was updated to accommodate the insourcing of post-acute care services. See Notable Changes for the updates, including codes that were removed from the list.

Pharmacy
2023 Medicare Cost-Sharing for Insulin and Part D Vaccines
Starting Jan. 1, 2023, the following changes will apply to our EmblemHealth and ConnectiCare Medicare Advantage plan (MAPD) and Prescription Drug Plan (PDP) members’ cost-sharing responsibilities:
- Members will pay no more than $35 for each one-month (30-day) supply of covered insulin. Members with extra help will pay less.
- Members will pay $0 for most covered Part D vaccines, including shingles vaccine.

Keep Your Directory and Other Information Current
Let Us Know When Directory Information Changes
If a provider in your practice is leaving, we ask that you inform us as soon as possible. To report other changes, sign in to your Provider/Practice Profile for EmblemHealth or ConnectiCare. If you participate with us under a delegated credentialing agreement, have your administrator submit these changes. See more on how to submit changes for EmblemHealth and ConnectiCare.
Remember to review your CAQH application every 120 days and ensure you have authorized EmblemHealth as an eligible plan to access your CAQH information.
Consult EmblemHealth’s Online Provider Manual for Important Information
The EmblemHealth Provider Manual is a valuable online resource and an extension of your Provider Agreement. It applies to all EmblemHealth plans and includes details about your administrative responsibilities and contractual and regulatory obligations.
