January is always a busy month for health care providers. Patients have new benefit plans or have reset benefit limits. Provider Portal: Small Change, Big Impact
We recently updated the process for uploading documents to an existing preauthorization in the provider portal. Now, when you select Add Supporting Documents, you will get a pop-up message asking if the information you are submitting will be used to help decide a pending or ongoing inpatient stay request or support discharge planning. Providing this information will help us take more effective action.
Reminder: Fax Lines To Be Disconnected May 2024
Starting May 1, 2024, we plan to disconnect the fax lines currently in use for preauthorization requests and concurrent reviews. Please start using the provider portal to submit your requests and supporting documentation. See the full announcements here: EmblemHealth | ConnectiCare.
Healthy Futures Program for Pregnant Members
Our EmblemHealth Healthy Futures Program is a maternity support program open to all eligible EmblemHealth members. Every provider working with pregnant EmblemHealth Medicaid, HARP, DSNP and Commercial patients should be aware of this important support program – starting during pregnancy and going through the first three months postpartum.
The program provides care management services, education, and support to expectant mothers, helping them manage their health and lifestyle during and after pregnancy. Members (ages 18-45) who are currently expecting or planning to start a family can access this program. There is no out-of-pocket cost to join.
If you have an EmblemHealth patient who may benefit, please encourage them to enroll by contacting our Healthy Futures team at 800-447-0768.
American Heart Month
February is American Heart Month and a great time to encourage your patients to practice a heart-healthy lifestyle. It is also a perfect time to engage patients who are managing cardiovascular disease.
The American Heart Association recommends statins of moderate or high intensity for adults with clinical atherosclerotic cardiovascular disease to help reduce the risk of heart attack or stroke. The following tips will help reinforce recommendations on statin use for patients with cardiovascular disease:
- Discuss with your patients why they are on a specific statin medication and explain the role and importance of statin therapy.
- Identify and help resolve patient-specific medication adherence barriers or concerns, such as adverse side effects or cost.
- Recommend mail order and/or 90-day prescription of maintenance drugs.
Statin therapy is only one aspect of care for patients with cardiovascular disease but it has a big influence on measuring overall effectiveness of care.
Cardiac rehabilitation can significantly improve functional status and other important health factors for patients who have experienced a cardiac event (heart attack, heart transplant, etc.). Cardiac rehabilitation includes exercise counseling and training, education for heart-healthy living, and counseling to reduce stress. It is widely covered by health plans and state and federal programs.
February is Children’s Dental Health Month
New parents often ask when their child should first see a dentist. The American Association of Pediatric Dentists recommends that parents establish a dental home for their child by their first tooth or first birthday.
The American Dental Association developed the February 2024 National Children’s Dental Health Month (NCDHM) Program Planning Guide with the theme “Brush, Floss, Smile.” This month-long national health observance brings together thousands of dedicated dental professionals, health care providers, oral health champions, and others to promote the benefits of good oral health and prevention to children and adults, caregivers, teachers, and many others.

Do Not Bill Members With Full Medicaid or Who Are Qualified Medicare Beneficiaries (QMB
If Medicare-Medicaid dual-eligible individuals have their Part A and Part B cost-share fully covered by their Medicaid plan or are QMBs, they are not responsible for their Medicare Advantage cost-share for covered services. Please do not balance bill these members for any other costs. Any Medicare and Medicaid payments for services given to these members must be accepted as payment in full.
For EmblemHealth members, you can use ePACES to check whether the member has full or partial Medicaid benefits. For more detail, see EmblemHealth Medicare Advantage Plans.

NY Medicaid, HARP, and Child Health Plus Updates
Updates to Dental Benefits for Medicaid and HARP Members
Starting Jan. 31, 2024, EmblemHealth will cover crowns and root canals in certain circumstances for Medicaid and Health and Recovery Plan (HARP) members. In addition, replacement dentures and implants will only need a dentist’s recommendation to determine if they are necessary.
The New York State Medicaid Program Dental Policy and Procedure Code Manual (Dental Manual) will be updated to reflect:
- Changes to the clinical criteria for coverage of root canals, crowns, replacement dentures, and dental implants to allow for expanded coverage of previously limited dental services.
- Revisions to the coverage policies for these dental services to expand coverage when medically necessary.
- Revisions to the prosthodontics policy that removes limitations on replacement prosthodontics coverage.
Antipoverty Program Targeting Expectant Mothers and Infants in NYC
In observance of Maternal Health Awareness Day, Jan. 23, and as part of EmblemHealth’s commitment to the health of pregnant women and infants, we are sharing an important resource with our providers that can help alleviate poverty and give this population a healthy start.
The Bridge Project (not to be confused with our Bridge Program) seeks to solve child poverty in New York. Launched in June 2021 by The Monarch Foundation as a 501(c)3 charity organization, it is New York's first consistent, unconditional cash allowance program for this population. The Bridge Project supports healthy development for babies during their first 1,000 days of life by providing their mothers with consistent cash on a biweekly basis for three years – no strings attached and no conditions.
The Bridge Project serves 1,100 babies and their mothers across all five boroughs of New York City and recently launched a presence across New York state, starting with Rochester and Buffalo.
The program will continue to expand as it works to solve child poverty here and across the country.
All expectant mothers who meet the below current criteria are encouraged to apply regardless of insurance coverage or citizenship status. Mothers are selected for the program from the pool of applicants and will be notified in their 23rd week of pregnancy if they will receive these unrestricted income payments.
The Bridge Project eligibility criteria*:
- Be 18 years old or older.
- Be 23 weeks pregnant or less with your first child.
- Have an annual household income under $52,000 for NYC residents, under $40,000 for Rochester and Buffalo
- Live in New York City, Rochester, or Buffalo
*Note: Eligibility criteria will be updated in 2024 as the Bridge Project expands.
Mothers can still receive a list of resources and organizations specific to their community if they do not meet the above criteria or meet the criteria but are not selected for the payments. In addition, every mother who completes a survey sent by the Bridge Project can receive up to $50 twice per year.

Change of Address and Contact Notification
Providers must notify Medicaid of any change of address, telephone number, or other pertinent information within 15 days of the change. For more information on this requirement and how to submit changes, see Reminder: Keep Your Directory Data Current.

Keep Your Patients Covered in 2024
Please remind your patients who are enrolled in Essential Plan 3 or 4 that they may need to submit additional information on their immigration status by their submission deadline to stay covered. Share this checklist with them. If they have questions, have them call us at 888-905-4974 (TTY: 711), 9 a.m. to 6 p.m., Monday through Friday.
If your patients need help in-person, they can stop by an EmblemHealth Neighborhood Care location.

New Formalized Pharmacy Reimbursement Policy
We have a cobranded Pharmacy Medical Claim Reimbursement Policy for EmblemHealth and ConnectiCare. The policy details guidelines and requirements to validate appropriate drug to therapy based on U.S. Food and Drug Administration (FDA) guidelines.
There are no changes to the policy criteria, only reformatted content transferred to a new template. Claim edits will be applied during claim processing.

New medical guidelines:
The Fetal Surgery medical guideline has been introduced for both EmblemHealth and ConnectiCare.
Revised medical guidelines:
- The EmblemHealth medical guideline for Dorsal Column Stimulator for Pain Management was updated to add Proclaim XR Spinal Cord Stimulation System to the list of limitations/exclusions as experimental/investigational for pain secondary to diabetic peripheral neuropathy.
- The EmblemHealth and ConnectiCare (commercial) medical guidelines for Mechanical Stretching Devices were updated to remove the pediatric use limitation (EmblemHealth | ConnectiCare).
- The EmblemHealth (commercial) and ConnectiCare medical guidelines for Infertility Services (EmblemHealth | ConnectiCare) were updated as follows:
- The Freeze-All Cycles section was updated to clarify that approval for a frozen embryo transfer (FET), as a continuation of the same in vitro fertilization (IVF) cycle, will be conditional on the preimplantation genetic testing being performed. In addition, a note was added stating that a current semen analysis is not required when FET is requested.
- Sections pertaining to Donor Services and IVF for Women without Male Partners or Exposure to Sperm added “storage” to the notes pertaining to noncovered expenses.
- The Fertility Preservation section was updated to add a note stating that fertility preservation services are a separate benefit to preserve fertility when a medical treatment will directly or indirectly result in iatrogenic infertility and do not count toward the three-cycle limit on IVF benefits.
- The Limitations/Exclusions section was updated to add Sperm-Hyaluronan Binding Assay (HBA) for selection of sperm for use with assisted reproduction technologies is experimental/investigational.

Let Us Know When Directory Information Changes
If a provider in your practice is leaving, please inform us as soon as possible. To report other changes, sign in to your Provider/Practice Profile for EmblemHealth or ConnectiCare. If you participate with us under a delegated credentialing agreement, please have your administrator submit these changes. Learn more about submitting changes for EmblemHealth and ConnectiCare.
Remember to review your CAQH application every 120 days and ensure you have authorized EmblemHealth as an eligible plan to access your CAQH information.
