HPV Screening Code Added to Provider Office Lab List

Effective July 9, 2016, code G0476 – HPV Screening, has been added to the EmblemHealth In-Office Testing List. This screening test is covered for female members between the ages of 30 and 65 years old, once every 5 years with one of the following diagnosis codes: v7381, v7231, z1151, z01411, or z01419.
