Non-contracted providers who submitted claims to EmblemHealth between June 2018 and November 2018 may have received an incorrect Electronic Remittance Notice (ERA) with the description code CO 45, indicating that the provider in question was participating. If you received an incorrect notice and need a corrected document, please contact our Customer Service Department to request an updated notice.
