Preauthorization List Updates – Bariatric Surgery

Retroactive to Jan. 1, 2023, the following codes have been removed from the EmblemHealth Preauthorization List for commercial members only. The Bariatric Surgery medical policy’s criteria continue to apply.

The following codes are removed from the EmblemHealth Preauthorization List starting Jan. 1, 2023.
