CHPlus New Member ID Card

The EmblemHealth Child Health Plus (CHPlus) member ID cards have been changed. Starting Jan. 1, 2023, primary care provider (PCP) referrals are not required for specialty care. Therefore, we have removed “Referral Required” on the upper right-hand corner below the plan name.
