In-Home Health Screenings by Signify

We are working in collaboration with Signify, a company staffed by licensed physicians and nurse practitioners, to provide selected Medicare members with an in-home screening so the member, and their provider, can better understand and manage their health needs. Signify’s in-home screenings can include a bone mineral density test of the heel.

Participation in this program is at the sole discretion of the member and does not affect their health plan coverage with EmblemHealth in any way. We are providing this value-added service free of charge.

Who is eligible for in-home screenings?

Medicare members selected for in-home screenings have had a fracture and have no recent history for a bone mineral density (BMD) test or prescription for a drug to treat or prevent osteoporosis.

What does Signify provide after the screening is completed?

All results and medical records from the visit will be sent to the member’s primary care provider (PCP) on record. Additionally, if the Signify clinician believes that the member would benefit from case management, they will inform the member's PCP and EmblemHealth.

How can you help?

We ask our PCPs to review the results received from Signify’s clinicians to identify opportunities to improve care. We also ask that our PCPs include Signify’s report in the member’s medical record

In-home visits are not meant to replace the care you provide. Rather, our goal is to support you and improve screening rates and health outcomes. Your patients will be encouraged to keep up with their preventive care and routine office visits.
