Care Management Services

The following policy is in accordance with the American Medical Association (AMA) CPT coding guidelines.

If coding for chronic care management (99490) and complex chronic care management services (99487 & 99489), the patient must have at least two chronic (continuous or episodic) conditions expected to last 12 months or until death. The care management service will be denied if billed without both a primary and a secondary diagnosis.
