Required Training for EmblemHealth Practitioners, Providers and Vendors
Each year, all Medicare providers are required to complete the Special Needs Plan (SNP) Model of Care (MOC) training for each of the Dual Eligible SNPs in which they participate, as mandated by the Centers for Medicare & Medicaid Services (CMS). Providers in the VIP Prime Network must complete the EmblemHealth SNP MOC provider training. Providers who do not complete the training will be removed from the EmblemHealth Provider Directory.Providers in the Medicare Choice PPO Network must complete training offered by GuildNet and ArchCare.
Use the links below to access EmblemHealth’s SNP Model of Care training. Large group practices can use the download version to review in a staff meeting, and then complete/return the attestation as instructed on the form. The online version also allows a designee on behalf of the group to attest that the training has been completed by the entire group.

Also annually, Associated Dual Assurance Network providers are encouraged to complete web-based Fully Integrated Duals Advantage (FIDA) provider training modules. To access the training site, visit:
Note to all Providers: Although offered in context of FIDA, the downloadable provider trainings are a useful resource for you and your staff regarding cultural competency, identifying and supporting patients with behavioral health issues and accommodating the disabled and the elderly.
