Our diverse team of Customer Care Navigators can help you build on healthy behaviors in your daily life and empower you to take control of your health.
Meet with our team to learn more about your health plan benefits or get help choosing a health plan that works for you. We offer support in English and Spanish.
You can also sign up for free fitness and wellness classes, health and nutrition workshops, and community events. Bring a friend – our classes are open to everyone!
At EmblemHealth Neighborhood Care Fordham Road, A1C blood sugar tests are available free of charge during operating hours.* They're also available during special diabetes screening** events as part of our Community-based Diabetes Wellness program.
