**Key features of our health coaching program* to help you get on track:**

Our health coaches offer support wherever you may need it by helping you set realistic goals and creating action plans to help you get there. Each session lasts about 25 to 30 minutes and you set the pace.
Easy communication plus online scheduling through your member portal or by calling EmblemHealth Medicare Connect Concierge at 877-344-7364 (TTY: 711). From April 1 to Sept. 30, you can call us from 8 a.m. to 8 p.m., Monday through Saturday. From Oct. 1 to March 31, you can call us from 8 a.m. to 8 p.m., seven days a week. We are happy to help you.
Join one-on-one coaching sessions by video or phone on topics ranging from how to cope with stress to healthy eating.

Can I switch my health coach if I need to?
Of course! You can change your coach through your member portal or by calling EmblemHealth Medicare Connect Concierge at 877-344-7364 (TTY: 711). From April 1 to Sept. 30, you can call us from 8 a.m. to 8 p.m., Monday through Saturday. From Oct. 1 to March 31, you can call us from 8 a.m. to 8 p.m., seven days a week. We are happy to help you connect with the coach that is right for you.

Does it cost anything to work with my health coach?
There is no out-of-pocket cost to you. Your Vitality WellSpark health coach is one of the many resources available to you through your Medicare Advantage plan.

How do I set my health goals?
During your first coaching session, you will have the opportunity to define the milestones that best meet your unique health needs and personalized goals. The good news is that you and your coach can check in regularly on progress and adjust as needed.

Can I cancel or reschedule an appointment?
Yes. If you need to cancel or reschedule an appointment, please call Vitality WellSpark Health at 877-224-7350 from 8 a.m. to 6 p.m., Monday through Thursday and 8 a.m. to 5 p.m. on Friday.

How to Schedule an Appointment With a Coach

Get the support you need to live a balanced, healthy life.

Sign in to the myEmblemHealth portal.
Click the 'Health and Wellness' tab at the top of the page.
Click 'Get Started' in the Wellness Program section.
If it’s your first time using Vitality WellSpark, you’ll need to agree to Terms and Conditions. Then, you’ll be sent to your new Vitality WellSpark benefits platform where you can enroll in health coaching.
Once you are logged in to the Vitality WellSpark platform, select the Benefits tab at the top of the screen, then Health & Lifestyle Coaching.
Answer the five questions so the coach can get to know you.
Agree to the coaching terms and conditions.
Follow the instructions and choose the appointment type that's best for you. You have the option of a phone coaching session or a video coaching session. A video coaching session requires access to a computer or smartphone. You will need to have an email address to receive directions.
Choose a coach. Our coaches have unique backgrounds and come from all walks of life. Choose the one that best fits your goals.
Pick a date and time for your appointment. Make sure to confirm your appointment by clicking the ‘Submit’ button. You will receive an email and/or text message confirmation once you have made a selection. Confirmations can only be sent to those with an email address or cell phone with text capabilities.
