The Hub provides a more seamless end-to-end journey for EmblemHealth members where teams provide comprehensive outreach unique to the member’s needs. Members receive fewer and more impactful outreach addressing multiple needs in a single interaction—from conducting welcome calls to completing health risk assessments (HRAs), addressing gaps in care, scheduling a visit to a PCP, and more.
