Wellness Program and Health Coaching

The Healthy Living questionnaire includes a series of questions about your overall health and wellness. It’s a useful way to measure and assess your starting point for achieving your best health. By completing the 20-25 minute Healthy Living questionnaire located on your EmblemHealth Wellness Program portal, you can have a coach contact you to start setting health goals. You can save your questionnaire at any point if you need to complete it later.
