What is the Diabetes Remote Monitoring Program?

The Diabetes Remote Monitoring program is a partnership between your AdvantageCare Physicians (ACPNY) doctor and the EmblemHealth Care Management team that aims to provide you with the resources and support you need to better manage your diabetes at home. This program is included in your EmblemHealth benefit package and is available to you at no additional cost.

If you join the program, your EmblemHealth Care Manager will work with you to create a personalized care plan that can help you meet your health goals. You will share your blood sugar levels and progress directly with your ACPNY doctor through your myACPNY account. Your Care Team will be able to see your blood sugar levels in real time and make better decisions to support your health.

Who is eligible to participate?

To qualify for this program, you must:

1. Be an EmblemHealth City of New York HMO or PPO member
2. Have a primary care physician at ACPNY
3. Have an active myACPNY account
4. Have a previous or ongoing relationship with EmblemHealth Care Management

What is included in this new service?

1. Outreach and education
2. Ongoing care management and coaching (if requested)
3. Resources for ongoing support and diabetes education (myACPNY portal, virtual series, classes, follow-up calls)

How long is the program?

Once enrolled, the program will last for 12 months. Once you have completed the program, you can still receive support services from your Care Manager and you will still have access to your health information in myACPNY.

How do I sign up?

For more information or to sign up for the program, please contact your EmblemHealth Care Specialist/Manager at 800-447-0768 (TTY: 711), Monday through Friday, from 9 a.m. to 5 p.m. They will complete an initial assessment to determine your specific health needs and work with you to address any concerns you may have. Your ACPNY doctor will then reach out to explain how you will work together using myACPNY.

Do I have to see my ACPNY doctor in person or can my visits be done virtually?

You can see your ACPNY doctor through a virtual visit or at your local ACPNY office. ACPNY is practicing social distancing and other important guidelines set by the CDC to keep everyone in our facility safe and healthy.
