New York, New York (November 9, 2017) – Today, EmblemHealth and Coalition of Asian-American IPA, Inc. (CAIPA) announced a new collaborative effort to provide Comprehensive Health Assessment services in the homes of Asian families. These Assessments focus on gathering a full picture of an individual’s health care needs to customize programs , resources and supports to improve health care interventions and the effectiveness of primary care.
CAIPA home health providers will be able to deliver culturally sensitive and linguistically competent services to home bound patients, while ensuring care coordination with the patients’ primary care physicians. The new service will give these patients time to discuss their complete medical history and any non-medical factors that can impact their well-being. Patients can also complete a comprehensive review of prescription and non-prescription medications, and discuss how nutrition and exercise can impact their health.
Those who have had Comprehensive Health Assessment services report extremely high satisfaction and more than 97 percent of patients would recommend it to others. CAIPA staff will conduct the Assessments using proven tools.
