This month, you will receive your Annual Notice of Change and Evidence of Coverage materials. Take time to read these items so you can better understand how your plan works and what services are available.

What is it?
The Annual Notice of Change explains changes in your coverage for 2018. Your Evidence of Coverage gives details about your coverage and how to get the services you need.

What should I do?
• Check the changes to your plan’s benefits and costs.
• Check to see if your doctors and other providers are in your plan’s network.
• Check to see if your prescription drugs will be covered.

Medicare Open Enrollment is October 15 to December 7. If you change your plan, coverage under the new plan will begin on January 1. If you want to change your plan, call Customer Service between October 15 and December 7.
